  • You should have good internet connectivity.

Upon completion of survey, you will be entered into sweepstakes for a chance to win $2,500 Saks Fifth Avenue Gift Card Quarterly.


Complete an accurate survey by following the given instructions:

  3. Click Next.
  4. Select the year and month of your birth.
  5. Select the store where you visited.
  6. Enter the TRM number.
  7. Enter the TRN number.
  8. Enter the Associate ID.
  9. Enter the date of your visit.
  10. The next step includes a series of questions. These questions are related to your experience at the store.
  11. Rate few situations according to your like and dislike.
  12. The last step is to submit the survey.
